Item # 1:   ( 255  -  40 )   The Department of Innovation and Technology intends to establish a 5-year master contract with a maximum potential spend of $60,000 for custom labels. Labels will be white polyester bar code labels, 1.75 inch wide x 0.625 inch tall (+/- 10%) with minimum .002 inch thickness, permanent MC78 Pressure-Sensitive adhesive, two colors, black/purple, with custom serialization. Quoted price will include all fees, to include (but not limited to), shipping and customization. Future orders must be delivered within 14 calendar days of vendor receiving an executed order. Spec sheet of offered product must be included with the vendor quote. The three lowest priced responsive vendors will be notified, and a sample label must be submitted within 14 days of notification.
