Provide Professional Services to conduct annual workplace surveys to receive detailed analytical reports, to gain insights and awareness to effectuate positive change for Department staff. Vendor shall provide quarterly workshops, training, or seminars to address the survey results. This information will also be used to enhance our Department's Diversity, Equity, Inclusion, and Accessibility (DEIA) plan. The contract has a term of upon contract execution for one (1) year. *** Vendor quotation is required to be valid for 180 days from date of submission. ***Vendor is to include all shipping and handling of any material that may sent to locations in the unit price ***Vendor to complete applicable Forms A or Forms B document and return with submission as an attachment ***Vendors required to submit all quotations electronically ***VENDOR NOTE - BACKGROUND CHECK - Revenue, at its expense, will require any Vendor employees and/or subcontractors personnel that will be 'on-site' to undergo and pass a criminal and driver history to include a State/Federal Security fingerprint background check. Vendor shall immediately reassign any individual who, found by the State's check, does not pass the background check. ***VENDOR - Please note: All Delivery and On-Site personnel are required to wear a face mask when performing any services at the Illinois Dept. of Revenue Facilities.
